The post holder will provide expert clinical expertise and operational management within the Virtual Wards (Hospital @Home) Model at Birmingham Community Healthcare (BCHC) NHS Foundation Trust.
The role involves working across various city-wide settings, including community and inpatient units, and providing in-reach into secondary care to identify and assess patients. The aim is to prevent avoidable hospital admissions and facilitate early supportive discharges. Collaboration with Secondary and Primary Care, Community Services, Urgent Care Response, Early Intervention Team (EICT), and Specialist Services is essential. The post holder will participate in multidisciplinary teams (MDTs) to offer advice, support, and data to support the Virtual Ward (Hospital @Home) Model.
You will join a community team amidst significant health and social care transformations, focusing on preventing unnecessary hospital admissions and enabling early discharge.
Create an environment where clients are central to their care and decision-making, respecting their dignity, wishes, and beliefs. Involve them in shared decision-making and obtain consent for treatments and referrals.
Ensure patient safeguarding by taking emergency actions when necessary, reporting risks immediately, and following safeguarding policies. Report safeguarding risks promptly within the team. The role may include working within the Virtual Ward Hub as a shift lead, triaging new referrals, allocating cases, monitoring patient progress, leading safety huddles, and conducting MDT case reflections.
