Job summary

East Lancashire Hospitals NHS Trust is seeking a clinically trained and experienced pharmacist to join the Pharmacy team. You will work within the clinical pharmacy service, including the Integrated Medicines Optimisation Team (IMOT), using a non-medical prescribing qualification to ensure patient's are continued to be prescribed their specialist medicines when they are discharged from hospital.

Qualifications

• MPharm degree or equivalent is required.
• Registered as a Pharmacist with GPhC.
• Post-graduate Diploma (PgDip) or equivalent preferred.
• Non-medical Prescribing Qualification desired.

Responsibilities

• Provide high quality pharmaceutical care within the Integrated Medicines Optimisation Team (IMOT).
• Act as a role model in clinical pharmacy and deliver high-quality pharmaceutical service.
• Support development of policies and procedures for safe medicines use within IMOT.
• Work with multidisciplinary teams to deliver integrated pharmacy services.
