Job summary

East Lancashire Hospitals NHS Trust is seeking a clinically trained pharmacist to join the Pharmacy team within the clinical pharmacy service and IMOT. The post-holder will use a non-medical prescribing qualification to ensure patients continuing on specialist medicines after discharge are managed appropriately.

The role requires close collaboration with consultants, specialist nurses, and outpatient staff, plus liaison with patients and guardians regarding ongoing medicines supply.

Qualifications

Responsibilities

Job description

An exciting opportunity has arisen for a clinically trained and experienced pharmacist to join the Pharmacy team at East Lancashire Hospitals NHS Trust. The post-holder will work within the clinical pharmacy service, including the Integrated Medicines Optimisation Team (IMOT), utilising a non-medical prescribing qualification to ensure patient's are continued to be prescribed their specialist medicines when they are discharged from hospital. The role will involve close working with members of the multidisciplinary team members, such as consultants, specialist nurses, outpatient pharmacy staff members, in addition to close liaison with patients or parents/guardians regarding their ongoing medication supplies. Main duties are summarised below.

• Provide high quality pharmaceutical care to patients within the Integrated Medicines Optimisation Team (IMOT)
• Act as a role model in the field of clinical pharmacy and provide a consistently high level of pharmaceutical service
