Job summary

Princess Alexandra Hospital NHS Trust (PAHT) is seeking an experienced pharmacist to deliver a high quality clinical pharmacy service on the Assessment Unit. You will work within a multidisciplinary team to provide a 7 day pharmacy service, supporting Ambulatory Unit and Surgical Assessment areas, and ensuring medicines reconciliation and optimisation for patients moving to wards.

Qualifications

Provide a high quality clinical pharmacy service on the Assessment Unit.
Work with the multidisciplinary team to deliver a 7 day pharmacy service.
Support Ambulatory Unit and Surgical Assessment areas.
Ensure medicines reconciliation and optimisation for patients moving to wards.
Deliver patient care through direct patient facing activities.
Develop guidelines and contribute to audit work.

Responsibilities

Liaise with prescribers, nurses and health care professionals to ensure efficient pharmacy service.
Undertake drug histories and medicines reconciliation for admitted patients.
Monitor prescriptions for safety, accuracy and cost effectiveness.
Counsel patients and carers about medicines and treatments.
Assist with Controlled Drugs management and stock checks.
Contribute to SOPs and education/training within the unit.
