Bradford District Care NHS Foundation Trust in Skipton is seeking a Band 7 Advanced Nurse Practitioner to join our Adult Community Mental Health Team. You will assess, plan, and manage care for adults with severe mental health needs across a rural patch, providing holistic support and prescribing where appropriate.

You will work autonomously within a supportive leadership team, with opportunities for professional development and progression within a trusted, values-driven organisation.

We are looking for a friendly, motivated, and passionate Band 7 Advanced Nurse Practitioner to come and join our well-established Adult Community Mental Health Team based in Skipton -- a beautiful rural market town often known as the gateway to the Yorkshire Dales. This is a really rewarding role, working across a wide and varied patch that includes both town and countryside communities. You'll be supporting people with severe and enduring mental health needs through assessment, formulation, care planning, risk management, and prescribing. Carry out physical checks prior to the prescribing, initiation and monitoring of medication, and also other medication where necessary. To work in a supported autonomous manner and to be able to plan and prioritise workload. To demonstrate effective networking skills to work in collaboration with a range of other services and external agencies. To actively participate in the flexible formats of team supervision and support. To work as part of the leadership team, providing support and cover for team leader when required.

• Experience of working with people with complex health needs
• Experience of instigating investigations, interpreting investigations and initiating appropriate plan of care for patients with Mental Ill health
• Experience of care co - ordination/ care management, Experience of working in the Community Mental Health care setting, Demonstrate a high level of clinical skill. Demonstrate an in depth knowledge and experience of clinical examination, history taking, pharmacology, pathophysiology etc
• Understanding of NHS Plan and implications for Primary and Secondary Care
