Sanctuary Personnel is recruiting a Band 6 Physiotherapist for the Discharge to Assess (D2A) Team in Basildon, Essex. The role is ongoing with flexible full-time or part-time hours, paying £26.00 per hour.

You will assess patients referred through the D2A pathway in their home environment, develop and review personalised treatment plans, and support discharge and rehabilitation to maximise functional independence.

Qualifications

• UK HCPC registered physiotherapist with degree/diploma in Physiotherapy.
• Post-registration CPD evidence is essential.
• Membership of the CSP is desirable.

Responsabilités

• Undertake comprehensive physiotherapy assessments for Discharge to Assess patients in home or community settings.
• Develop, implement and review individualised treatment and rehabilitation plans to maximise functional independence.
• Assess mobility, falls risk, environmental factors and equipment needs, arranging provision where appropriate.
• Work autonomously while managing a complex caseload of frailty and comorbidities.
• Collaborate with multidisciplinary team colleagues including OTs, nurses, social workers, GPs and community services.

What you will do:

• Undertake comprehensive physiotherapy assessments for patients referred through the Discharge to Assess pathway within their home environment.
• Develop, implement, and review individualised treatment and rehabilitation plans to maximise functional independence and wellbeing.
• Assess mobility, falls risk, environmental factors, and equipment requirements, arranging provision where appropriate.
• Work autonomously while managing a complex caseload of patients with frailty, long-term conditions, and multiple co-morbidities.
• Collaborate effectively with multi-disciplinary team colleagues, including Occupational Therapists, Nurses, Social Workers, GPs, and community services.

Person Specification:

Qualifications:

• Degree or Diploma in Physiotherapy recognised within the UK, with current registration with the Health and Care Professions Council (HCPC).
• Evidence of relevant post-registration Continuing Professional Development (CPD) is essential.
• Membership of the Chartered Society of Physiotherapy is desirable.

Registration & Compliance:

• Must be an HCPC Registered Physiotherapist.

Experience:

• Prior experience as a Band 5 or Band 6 physiotherapist within acute, rehabilitation, frailty, or community services is required.
• Experience in discharge planning and supporting patients with complex health needs is essential.
