Job summary

NHS Professionals is seeking a Band 6 Physiotherapist to join the Community team based at Wokingham Community Hospital, Berkshire.

The role involves working within the Intermediate Care Team to assess patients referred to intermediate care services and develop care plans in partnership with the patient and MDT.

Qualifications

• Essential: Physiotherapy qualification and experience working with the elderly mandatory.
• Essential: HCPC registration mandatory.
• Desirable: RiO training desirable.

Responsibilities

• Assess and provide high standards of initial assessment input to patients in the community.
• Liaise with MDT members in treatment and management care plans and work closely with therapists.
• Create and evaluate care plans; discharge planning and further referrals as required.
• Devise physiotherapy treatment plans with patient, relatives, carers and team.
• Use and report on standardised and non-standardised assessments to diagnose and analyse complex patient needs.

Are you an experienced Physiotherapist looking for a Community role? We have a new and exciting opportunity for a Band 6 Physiotherapist to join the team based in Wokingham.

You will be a practitioner with the clinical skills and expertise and knowledge to support the pathways that sit under the Intermediate Care service: Community physio, Falls, Reablement /Urgent Community Response. You will work within the multi-disciplinary integrated team to jointly assess and plan the care of people who are referred to intermediate care services.

Main duties of the job

• Be able to assess and provide high standards of initial assessment input to patients in the community.
• Be able to liaise with MDT members in treatment and management care plans and working closely with therapist.
• Create, evaluate all care plans implemented and should have a sound knowledge in discharge planning and ability to make further referrals in to the community as required.
• Devise Physiotherapy treatment plans and goals in collaboration with the patient, relatives, carers and the interdisciplinary team.
• Be highly skilled in using and reporting on standardised and non-standardised Physiotherapy assessment to diagnose and analyse highly complex patient needs in the context of activities of daily living.
