Job Overview:
We are seeking an experienced Band 6 Occupational Therapist to join the team at Manchester University NHS Foundation Trust. This role involves providing specialist assessment and discharge planning support in an acute medicine environment. Key Responsibilities:
Conduct comprehensive assessments to identify patients' needs within the acute medicine setting

Plan and implement effective discharge strategies in collaboration with patients, families, and multidisciplinary teams

Provide expert advice on rehabilitation and ongoing care plans to ensure seamless transitions
*
Maintain accurate and up-to-date documentation in line with Trust standards
*
Participate in team meetings, case reviews, and service improvement initiatives
Requirements:
HCPC registration as an Occupational Therapist

Proven experience in acute medicine, focusing on assessment and discharge planning
*
Strong clinical reasoning and decision-making skills
*
Excellent communication and collaboration abilities within multidisciplinary teams
*
Commitment to delivering patient-centred, high-quality care
