Job summary

Sanctuary Personnel is seeking a Band 6 Occupational Therapist to join the Community Specialist Practitioner team in Cambridge. This full-time role focuses on specialist discharge planning and rehabilitation within the Intermediate Care Service for older people and adult care.

You will conduct specialist assessments, coordinate discharge pathways, mentor junior staff, and provide clinical advice to patients and carers.

Responsibilities

• Complete specialist assessment, rehabilitation, and discharge planning for complex patients.
• Undertake clinical triage of referrals into Intermediate Care services.
• Complete trusted assessor assessments to support patient care.
• Assess, order, and review specialist equipment for patient needs.
• Coordinate complex discharge pathways from acute and community settings.
• Manage a specialist caseload and coordinate patient flow efficiently.
• Lead case conferences to support safe and timely discharge.
• Provide specialist clinical advice to patients, carers, and professionals.
• Support admission avoidance and facilitate earlier discharge.
• Make recommendations regarding healthcare packages and onward referrals.
• Liaise with multidisciplinary teams, community services, and care providers.
• Maintain accurate clinical records and documentation.
• Support service development, audits, and quality improvement initiatives.
• Mentor, supervise, and appraise junior staff members.
• Deliver training on discharge planning and Intermediate Care pathways.
