Job summary

Sanctuary Personnel is seeking a Band 6 Community Specialist Practitioner - Occupational Therapist in Cambridge for our Intermediate Care Service. This full-time role focuses on specialist discharge planning and rehabilitation for older people, providing high-quality community care and support.

You will complete specialist assessments, coordinate complex discharge pathways, mentor junior staff, and liaise with multidisciplinary teams to promote timely, safe patient discharge.

Qualifications

• Must be a registered Occupational Therapist with Band 6 level experience.
• Experience in community discharge planning and intermediate care settings preferred.
• Ability to mentor junior staff and deliver training on discharge pathways.

Responsibilities

• Complete specialist assessments, rehabilitation, and discharge planning for complex patients.
• Undertake clinical triage of referrals into Intermediate Care services.
• Lead case conferences to support safe and timely discharge.
• Coordinate complex discharge pathways from acute and community settings.

• Provide specialist clinical advice to patients, carers, and professionals.
• Support admission avoidance and facilitate earlier discharge.
• Make recommendations regarding healthcare packages and onward referrals.
• Liaise with multidisciplinary teams, community services, and care providers.
• Maintain accurate clinical records and documentation.
• Support service development, audits, and quality improvement initiatives.
• Mentor, supervise, and appraise junior staff members.
