Band 6 Nurse Care Coordinator (Specialist Practitioner) - Swindon

Join to apply for the Band 6 Nurse Care Coordinator (Specialist Practitioner) - Swindon role at Avon and Wiltshire Mental Health Partnership NHS Trust.

The role involves care coordination for Service Users, including assessments and delivering interventions to individuals, families, and groups. You will work alongside clinicians such as family workers, CBT therapists, psychiatrists, and psychologists.

Driving is essential as supporting clients to appointments is part of the role. Flexibility in working hours may be required to reach clients and carers. You will be part of a small MDT team with opportunities for training in psychological interventions and shared learning across teams.

Key Responsibilities
  • Conduct comprehensive mental health assessments using tools like CAST, KGV, and Recovery Star.
  • Assess history, strengths, aspirations, mental state, cultural impact, functional needs, family and carer needs, risk, physical health, and social care needs.
  • Evaluate capacity under the Mental Capacity Act and plan interventions accordingly.
