Sanctuary Personnel is seeking a Band 6 Physiotherapist to join the Community Team in Thatcham. You will conduct comprehensive assessments, create individualized rehabilitation plans, and manage your own caseload in a supportive multidisciplinary setting.

The role offers an ongoing full-time position with an hourly rate up to £26.00 and includes opportunities for professional development and community-focused care delivery.

Qualifications

• Degree or Diploma in Physiotherapy.
• HCPC Registration.
• NHS Physiotherapy experience essential.
• Community Physiotherapy experience preferred.
• Strong assessment, rehabilitation and clinical reasoning skills.
• Experience managing an independent caseload.
• Full UK driving licence and access to a vehicle.
• Excellent communication and multidisciplinary working skills.
• Community Rehabilitation experience and Intermediate Care experience.

Responsabilités

• Undertake comprehensive Physiotherapy assessments within community settings.
• Develop and deliver tailored rehabilitation and treatment programmes.
• Manage an autonomous caseload effectively and efficiently.
• Assess mobility, functional ability and rehabilitation needs.
• Support discharge planning and transitions back into the community.
• Contribute to community rehabilitation pathways and promote independence.
• Work with multidisciplinary teams to deliver coordinated patient care.
• Review treatment plans and adapt interventions to progress.
• Maintain accurate clinical records and documentation.
• Provide advice and support to patients, families and carers.
