Job description

Job Title: Band 6 Community Nurse

We are currently recruiting for a Band 6 Community Nurse for our client based in the Stoke-on-Trent area, on a 3-month contract.

Working Hours: Monday to Sunday, 9am-8pm, on the Home First team.

Responsibilities:

1. Take a lead role in the clinical assessment of individuals in their own homes.
2. Provide advice, support, and guidance to colleagues and other professionals regarding Home First services.
3. Coordinate patient care effectively.
4. Inform the manager of any issues impacting the Home First team.
5. Manage Home First referrals in accordance with referral criteria.
6. Participate in induction, training, and support activities for team members.

Rates:

• Monday-Friday Days: £26 per hour
• Saturday/Nights: £34 per hour
• Sundays/Bank Holidays: £41 per hour

Essential Requirements:

• Registered with the NMC
• Previous nursing experience
• Valid car driver’s license
