• Strong skill base to provide high quality care for patients.

Responsabilités

• Establish effective communication networks with patients, carers and multidisciplinary teams to support discharge planning.
• Build professional relationships with patients and carers, respecting views, autonomy and culture.
• Communicate sensitive rehabilitation outcomes and discharge challenges to patients and families.
• Attend ward/board rounds, MDT meetings and case conferences to update patient programmes.
• Demonstrate rehabilitation and manual handling techniques to relatives and carers prior to discharge.
• Liaise with Dorset Integrated Health and Care and other partners for referrals.
