This is a rewarding opportunity to contribute to improving the lives of older people while developing your skills within a supportive and dedicated multidisciplinary team.

The post holder will support the assessment, planning and delivery of person-centred care under the guidance of a supervising clinician, ensuring care is provided in line with clinical governance standards. They will undertake and accurately record physical health checks, including supporting individuals during clozapine titration and promoting their overall physical wellbeing.

The role also includes leading on carer support within the team, ensuring carers are identified, involved and provided with appropriate information and support while individuals are receiving care from the service.
