Lincolnshire Partnership NHS Foundation Trust is seeking a Senior Mental Health Practitioner (Registered Mental Health Nurse, Occupational Therapist or Social Worker) to support patients at The Deepings Practice and Bourne Galletly Practice in Stamford. The role requires delivering recovery-focused, person-centred care within primary and secondary care settings.

Qualifications

• Essential degree in mental health and social care or equivalent.

Responsibilities

• Provide specialist mental health assessments for patients registered with GP practices.
• Deliver recovery-focused interventions to promote independence and wellbeing.
• Develop holistic, person-centred care plans with patients and carers.
• Collaborate with secondary mental health services and neighbourhood teams.
• Attend and contribute to MDT meetings and case conferences.
