Job summary

Lincolnshire Partnership NHS Foundation Trust is seeking a Band 7 Mental Health professional to work within Meridian PCN, delivering mental health support at the primary care level and coordinating with secondary care as needed.

The role promotes hybrid working, engagement with GP surgeries, and guiding service users to the right mental health care. Applicants should hold relevant qualifications and be prepared for cross-organisational collaboration.

Qualifications

• Degree in mental health and social care or equivalent
• Evidence of advanced clinical practice at post graduate level to masters or equivalent
• Evidence of continuous professional development
• Management or leadership qualification
• Non-medical prescriber (or ability to complete within first 12 months in post)

Responsibilities

• Offer consultations within Meridian PCN surgeries to meet mental health needs at primary care stage.
• Provide intervention, mental health support and advice to the population and link with secondary care when appropriate.
• Collaborate with secondary care to ensure alignment with NHS Transformation 10 year plan.
