Job summary

Cardiff and Vale University Health Board in Stockport is seeking a Senior Clinical Assessor/Navigator to lead care navigation and digital remote monitoring for acutely unwell adults referred via urgent care. The role combines clinical assessment with digital health tools to identify deterioration and escalate to senior clinicians.

You will work within the Digital Hub, complete electronic records using Adastra, and support referrals to the correct service.

Qualifications

  • 1st Level Registered Nurse required.
  • Degree qualification or experience to demonstrate level of working, Advanced clinical skills or working towards.
  • Urgent care / emergency room experience in a community or hospital setting.
  • Evidence of continuing professional development.
  • Able to demonstrate leadership development and experience of digital monitoring of patients.
  • Experience of autonomous decision making and managing change.
  • Experience of managing a team and day-to-day workload.
  • Excellent written and oral communication skills and planning of staffing resources.

Responsibilities

Job description

The newly created Digital Health Service will be responsible for the clinical assessment and monitoring of Hospital at home patients alongside the senior clinical assessment and care navigation of the acutely unwell adult that has been referred via the Urgent Single Point of Access. The Senior Clinical Assessor/Navigator role will have dual function: Care Navigation and identification of clinical service for any acutely unwell adult referred Clinically assess and offer digital remote monitoring of Hospital at home patients upon the green caseload, identifying deterioration and managing escalation to the Senior Clinicians in VW The role will require clinical skills of assessment, alongside the ability to utilise digital equipment and telephony systems to support the role within the Digital Health Service., Collaborate with referring HCP and provide the appropriate delivery of patient care and clinical and professional advice that will result in effective navigation of the patient to the appropriate service Develop clinical skills in the assessment of acutely ill patients via the Digital, remote technology. Provide the delivery of evidence based nursing care by providing expert clinical assessment, care planning, care delivery, care evaluation and clinical advice in accordance with NMC Code of Professional Conduct. The post holder will work as part of the Digital Hub team and after accepting the referral will ensure that the correct triage tool is utilised to support effective care navigation to identified services within Stockport. The service Standard Operating Procedure will be adhered to alongside use of the Directory of Service DOS The post holder will ensure the patient is navigated to the correct department and team ensuring electronic records are contemporaneously completed and referral is accurately transferred. The post holder will be responsible for direct bookable appointments into identified services via Adastra and be compliant with the protocol and guideline for its use. If appropriate will signpost the referrer or patient to appropriate alternative services.   • 1st Level Registered Nurse.
  • Degree qualification or experience to demonstrate level of working, Advanced clinical skills module or equivalent or working towards.
  • Urgent care / emergency room Experience/community speciality or educational qualification, Has specialist knowledge of a broad range of medical conditions and treatments within sphere of responsibility.
  • Evidence of continuing professional development.
  • Able to demonstrate leadership development, Evidence/ experience of digital monitoring of patients, Experience of responding to sudden and unplanned changes in medical conditions.
  • Experience of making autonomous decisions
