Job summary

You will manage a complex caseload, formulate and progress individualised treatment plans, promote independence and support safe transfers of care across inpatient and community settings.

The post rotates every six months through diverse settings, with 37.5 hours per week and shift patterns including weekend work.

Qualifications

• Autonomous practitioner with ability to lead patient care decisions.
• Ability to work within a multidisciplinary team and coordinate care.
• Proven clinical reasoning and decision-making skills.
• Commitment to audit, evaluation and CPD activities.

Responsibilities

• Manage a complex caseload and progress treatment plans.
• Promote patient independence and reduce deterioration or admissions.
• Support safe transfers of care and communication across settings.
• Contribute to audit, evaluation and service development.
