Job summary

Oxleas NHS Foundation Trust in Woolwich is recruiting an experienced Occupational Therapist to lead autonomous community rehabilitation. You will assess, plan, implement and monitor client care with a strong emphasis on evidence-based practice and clinical reasoning.

You will work within a multidisciplinary team, supervise OT support staff, and contribute to service development and audits while delivering patient-centred care across long-term neurological conditions.

Qualifications

• Experience with adults with neurological and/or long-term conditions.
• Strong clinical reasoning and communication skills.
• Ability to work autonomously within the community and in partnership with MDT.

Responsibilities

• Provide OT assessment as autonomous practitioner in community settings.
• Develop individual care plans using evidence-based practice.
• Obtain client and/or carer consent per guidelines.
• Manage a clinical caseload including review and discharge decisions.
• Maintain up-to-date records and write discharge reports.
• Supervise OT support staff and students on fieldwork.
• Contribute to service development and research/audit activities.
• Collaborate with neurology specialists and Neurorehabilitation teams.
