Job summary

Birmingham Community Healthcare NHS Foundation Trust seeks an experienced clinician for a Band 6 role in its Single Point of Access service. You will triage and assess patients with acute and long-term conditions, operating autonomously within agreed pathways on a seven-day rota.

• Demonstrated experience in complex clinical triage and assessment.
• Ability to identify red flags and escalate concerns promptly.
• Experience working within NHS pathways and governance.

Responsibilities

• Undertake complex clinical triage and assessment using a range of consultation methods.
• Take clinical histories, identify red flags and elevate concerns promptly.
• Use clinical judgement and evidence-based triage tools to choose the care pathway.
• Initiate investigations and interpret results, planning and evaluating care with senior support.
• Make referrals for medical assessment and diagnostic procedures within agreed pathways.
• Signpost and refer patients to health and social care services to reduce A&E attendances and admissions.
• Support safe discharge into community settings.
• Communicate complex information clearly to patients, carers and professionals.
