Job summary

The Countess of Chester Hospital NHS Foundation Trust is seeking a permanent Advanced Clinical Practitioner to work within the Same Day Emergency Care (SDEC) and Emergency Department footprint at the Countess of Chester Hospital.

The ACP will work with the multidisciplinary team and autonomously to provide clinical care, expertise and advice to support same day discharge and prevent unnecessary admissions. A background in Frailty would be advantageous.

Qualifications

• MSc Advanced Practice/Clinical Masters with evidence of clinical competencies sign off
• HCPC/NMC registration (AHP or Nursing)
• Continuous Professional Development portfolio and relevant clinical skills
• Trained in Intermediate Life Support
• Recognised teaching/assessing qualification

Responsibilities

• The ACP role operates beyond traditional boundaries, providing autonomous and team-based advanced clinical care
• Support same day discharge and prevent unnecessary admissions in the SDEC/ED footprint
• Offer clinical advice to patients, carers and healthcare professionals within local guidelines
