Job summary

We have an exciting opportunity for an experienced Advanced Clinical Practitioner (ACP) to join the team at Newquay Community Hospital, delivering high-quality, patient-centred care within a community hospital setting.

This autonomous role involves managing patients with acute and chronic conditions, supporting admission avoidance, rehabilitation, patient flow and safe discharge planning. The ACP will provide expert assessment, diagnosis, treatment and prescribing within scope of practice.

Qualifications

• Masters in Advanced Nurse Practitioner or verified portfolio of evidence.
• Independent Non-Medical Prescriber Qualification with NMC registration.
• Recognised teaching or mentorship qualification equivalent to level 3.

Responsibilities

• Provide expert autonomous clinical care within Newquay Community Hospital.
• Advanced assessment, diagnosis, treatment and management of patients with acute, chronic and complex conditions.
• Independent prescribing within scope of practice.
• Develop care pathways for older people and those with multiple long-term conditions.
• Lead quality improvement initiatives and support education of colleagues.
