Qualifications

  • Registered Nurse / Paramedic / Allied Health Professional registration is required.
  • Strong clinical assessment and decision-making skills are essential.

  • Confident in seeing patients of all ages.

Responsibilities

  • Provide high-quality clinical assessment, diagnosis and treatment.
  • Manage a caseload of patients independently.
  • Conduct a mix of face-to-face and telephone consultations.
  • Undertake home visits where clinically necessary.
  • Work collaboratively with the general practice team to meet patient needs.

  • Support delivery of policy and procedures and provide nurse leadership as required.

Job summary

We are seeking an enthusiastic Advanced Care Practitioner to join our friendly, patient-centred and forward thinking team based in Louth.

The successful applicant will work autonomously to assess, diagnose and treat patients across all age groups, providing high-quality clinical care both within the practice and in patients homes where required.

Main duties of the job
  • Provide high-quality clinical assessment, diagnosis and treatment.
  • Manage a caseload of patients independently.

  • Maintain accurate clinical records using SystmOne (training available if required).
  • Contribute to practice initiatives and continuous improvement.
