Job summary

Cheshire and Wirral Partnership NHS Foundation Trust in Macclesfield is seeking an Outreach and Support Worker for its Adult Autism Service. The role is based at Beech House, Macclesfield, and supports adults with autism or those awaiting an autism assessment.

You will work with autism hubs and third-sector partners to deliver community-based support under qualified practitioners, with follow-up interventions. This part-time post is 22.5 hours per week across 3 days.

Responsibilities

• Work under the direction of qualified practitioners providing follow-up support to bespoke interventions.
• Provide basic information, advice, guidance and training to other health professionals supporting autistic adults.
• Work to reduce the barriers faced by autistic adults when accessing mainstream services and encourage the knowledge base and skill set of other commissioned services in providing reasonable adjustments in line with the Equality Act.
