Job summary

Active Care Group’s Brownbill Case Management division seeks an Associate Case Manager to support complex packages of care and coordinate rehabilitation and equipment needs. A UK professional health qualification (HCPC/NMC/Social Worker) and driving licence are essential, with 3+ years in case management and willingness to travel across multiple locations.

Qualifications

At least 3 years experience within a Case Management role
UK professional health qualification (HCPC, NMC or Social Worker Registered)
Full Driving Licence and access to a car
Experience supporting people with neurological conditions in a community setting
Knowledge of CQC fundamentals and DoLS/Safeguarding frameworks

Responsibilities

Advocating for the Client’s needs and building relationships with families and solicitors
Produce Initial Need Assessments, action plans and SMART goals
Recruit, select and manage care/support workers for Clients
Implement an individual client-focused Support & Rehabilitation Plan
Coordinate MDT professionals to deliver rehabilitation
Monitor and review Case/Support Plans and adjust as needed
Prepare risk assessments and manage safety measures
