Brownbill Case Management, part of the Active Care Group, is seeking Associate Case Managers to support specialist case management services for individuals with neurological and complex health needs. The role requires driving and travel across regions, plus HCPC/NMC or social work registration.

You will coordinate care packages, recruit support teams and develop rehabilitation plans. The position offers competitive hourly rates and access to expert clinical leadership.

Qualifications

  • Hold UK professional health qualification with HCPC/NMC or social work registration.
  • Full driving licence and access to a car.
  • Experience supporting people with neurological conditions in a community setting (adults or children).

Responsibilities

  • Advocate for clients' needs and build strong relationships with families, deputies and solicitors.
  • Produce Initial Needs Assessments, action plans and SMART goals with timeframes.
  • Recruit, select and manage a team of care/support workers for clients.
  • Implement an individual, client-focused Support & Rehabilitation Plan with guidelines for the team.
  • Coordinate MDT professionals to deliver the rehabilitation plan and monitor progress.
  • Compile risk assessments and identify risk management strategies.

Job description

Brownbill Case Management has been established since 1992 and exists to deliver an outstanding case management service to children, young people and adults who have enduring and complex health conditions and injuries.As part of the Active Care Group (ACG) Case Management Division, Brownbill Case Management Services (BAL) provides a specialist case management service primarily for individuals who have been affected by brain injury, spinal cord injury, Cerebral Palsy, complex orthopaedic injuries and amputation. We work across the Midlands, London and Home Counties, and South East regionsWe are currently interested in Associate Case Managers to support our specialist case management services, including assisting with arranging complex packages of care, coordinating rehabilitation and healthcare, procuring specialist equipment, and facilitating educational, vocational and leisure opportunities.For this role you will need to be a driver with ability to travel and hold a UK professional health qualification which requires you to be HCPC, NMC or Social Worker Registered. It would also be an advantage if you are an Advanced Members or working towards advanced membership status of the British Association of Brain Injury Case Managers (BABICM).We can offer competitive hourly rates and the support and guidance of the Brownbill team.What you'll be working:We can consider both full-time and part-time (minimum of 3 days p/wk preferred)Our coverage is London & Home Counties, Midlands, Kent, Essex and the South Coast, candidates from these areas would be highly desirable*Please note this can be discussed further*What you'll be doing:Advocating for the Client’s needs (both adults and children) while building strong working relationships with families, deputies and litigation solicitorsProduce Initial Need Assessments, action plans and SMART goals with timeframes that are specific and relevant to the clientRecruit, select and manage a team of care/support workers for ClientsImplement an individual client focused Support & Rehabilitation Plan with specific guidelines for the support teamCompile and manage MDT team of required professionals to deliver the rehabilitation planMonitor and review the Case Management Plan and Support & Rehabilitation Plan, amending as per the Client’s needsCompile risk assessments on key activities as required and identify risk management strategies that will manage the safety of all parties in a positive mannerWhat you'll have:At least 3 years experience within a Case Management roleHold a UK professional health qualification which requires you to be HCPC, NMC or Social Worker RegisteredFull Driving Licence and access to a carExperience supporting people with neurological conditions in a community setting - this could be adults or childrenThorough knowledge of the CQC fundamental standards and key lines of enquiryFull working knowledge of the Mental Capacity Act (MCA), Deprivation of Liberty Safeguards (DoLS) and Safeguarding frameworksAbility to address and respond to employee issues in a fair and consistent mannerStrong organisational, time management and planning skillsFlexible, professional and positive approach to multi-site workingWe also want to ensure that individuals with disabilities are provided reasonable facilities to participate in job applications or interviews to perform essential job functions.
