Brownbill Case Management Services (BAL), part of the Active Care Group, is recruiting a Clinical Case Manager to coordinate cost-effective rehabilitation and support for clients with complex health needs. Brownbill Case Management has been established since 1992 and exists to deliver an outstanding case management service to children, young people and adults who have enduring and complex health conditions and injuries. As part of the Active Care Group (ACG) Case Management Division, Brownbill Case Management Services (BAL) provides a specialist case management service primarily for individuals who have been affected by brain injury, spinal cord injury, Cerebral Palsy, complex orthopaedic injuries and amputation. We work across the Midlands, London and Home Counties, and South East regions.
The primary role of the Clinical Case Manager is to network cost effective services and resources on behalf of our clients to bring about an improvement in their quality of life. This includes assessing, facilitating, planning and advocating for their needs on an individual basis. This ensures that management is Client focused; that the most appropriate services are accessed; and that continuity, consistency and structure are maintained. Our work includes setting rehabilitation goals with Clients and support workers in the home environment; facilitating the client's and family's adjustment to the injuries; recruiting, training and supporting support workers; and managing care regimes.
Ideally: Full time, 35 hours per week / negotiable for an Associate Case Manager Mon to Fri office hours Hybrid. Our coverage is London & Home Counties, Midlands, Kent, Essex and the South Coast, candidates from these areas would be highly desirable. Please note this can be discussed at the interview.
Brownbill work closely with the client and their family or representative to provide tailored support packages for clients in their own homes and in the community. Brownbill has continued to expand to meet the increasing requirement for case management services and now boasts a multi-disciplined team of 10 case managers plus support staff at head office and manages over 250 care/support workers on behalf of Clients. Brownbill is determined to continually develop in order to provide intervention for as many people as possible.
