Job summary

Lincolnshire Community Health Services NHS Trust is seeking professionals to independently assess, treat and discharge individuals using the Home First approach. You will develop rehabilitation plans addressing cognitive, psychological, social, sensory and medical needs, supporting community independence with a person-centred method.

Qualifications

• Experience delivering rehabilitation plans within a health and social care setting.
• Ability to work in a multi-disciplinary team and liaise with safeguarding teams.
• Knowledge of community discharge planning and patient flow improvement.

Responsibilities

• Assess, treat and discharge individuals using Home First approach.
• Develop graded rehabilitation plans addressing cognitive, psychological, social, sensory, perceptual, motor and medical needs.
• Support independence in the community with a person-centred approach.
• Collaborate with carers, relatives and other professionals to holistically assess needs.
• Provide information on community services to promote self-care and access.
• Maintain caseload with supervision and guidance from a registered practitioner.
• Support acute and community hospital discharge planning for transitions to Adult Community Therapy.

Independently assess, treat, review and discharge individuals utilising the Home First approach and work towards supporting people to meet their goalsTake account of a persons cognitive, psychological, social, sensory, perceptual, motor and medical aspects whilst developing a rehabilitation plan with the individual which is graded appropriatelySupport individuals to remain independent in a community setting utilising a person centred approach whilst taking account of the enablers, carers and relatives opinions.Work within a multi-disciplinary team of professionals to ensure peoples needs are holistically assessed and addressedReview and monitor plans with individuals and their support networks, making appropriate adjustments as necessaryProvide information on community services available to promote self-care activities and provide advice and support as required to access these servicesProtect individuals and their support network from harm and abuse as far as possible, liaising with other professionals and the safeguarding team whenever there are concernsMaintain a caseload of patients working alongside own competencies, experience and training whilst receiving supervision and guidance from a registered practitioner to develop skills and support with more complex casesTo support both acute and community hospital with discharge planning for individuals who will be transitioning into the care of Adult Community TherapyTo support the service in co-ordinating project development to improve patient flow by supporting and communicating with alternative services within the health and social care sector
