The salary scale above has been agreed as part of the NHS Agenda for Change pay award for 2024/2025 and will be implemented in November 2024 backdated to 1st April 2024 where applicable.

Job overview

A new and innovative role based within Community hospital; the post holder will work as part of a designated Team (which includes Ward Manager, Advanced Nurse Practitioners/Nurse Practitioners), delivering evidence-based high-quality clinical care that focuses on the direct needs of the patient including those individuals with chronic conditions.

The post holder will work under the direction and support of a Registered Nurse by following the Health Board’s protocols, policies, procedures, pathways, clinical governance requirements, and care plans.

Main duties of the job
  1. Ensure effective communication with patients, carers, multidisciplinary team and the wider health and social care community.
  2. Practice in accordance with Health Board standards and statutory requirements and operate within the boundaries of the role and assessed competencies.
  3. Use personal effectiveness and knowledge to contribute to the development and delivery of a quality service.
  4. Work proactively as a member of the multi-disciplinary and multi-professional healthcare team, in support of registered practitioners.
  5. Manage a small defined caseload of patients with chronic conditions under the supervision of the Ward manager/registered nurse and/or nurse practitioner.
  6. Administer within the scope of competence prescribed medication/medicinal products where designated according to BCUHB policy.
  7. The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
