This role involves advanced holistic therapeutic assessments across a diverse range of physical and psychological conditions, holding your own caseload and ensuring the client record on Systm1 reflects the full journey promptly.

Job description

To provide advice, teaching and instruction to relatives, carers and other professionals to promote the understanding of the aims of therapy and to ensure a constant approach to patient care. To undertake advanced holistic therapeutic assessments with a diverse range and complexity of physical and psychological conditions which increase the risk of falls. To hold own caseload of clients referred. Taking responsibility for accepting and discharging these patients, undertaking all aspects of clinical duties required within the pathway as an autonomous practitioner. To ensure the client record on Systm1 contains the clients full journey in a timely manner.
