Job summary

East London NHS Foundation Trust is seeking an Assistant Practitioner to work in a multidisciplinary team across primary care settings, including clients’ homes, residential homes and clinics, delivering a range of therapeutic and clinical interventions based on the team’s skill mix.

You will participate in assessment, planning and implementation with guidance, identify suitable assistive equipment, work autonomously in the community, travel as needed, and maintain clear records while promoting

Responsibilities

  • Work in a multidisciplinary primary care team including homes and clinics, supporting diverse therapeutic interventions.
  • Participate in assessment, planning, implementation and individualized therapeutic interventions under guidance.
  • Identify and provide appropriate assistive equipment to improve client safety at home.
  • Work autonomously in the community; cover shifts including weekends and bank holidays.
  • Travel across the county as service demands require.
  • Form professional relationships with clients, families and carers, communicating respectfully and supporting autonomy.
  • Involve clients and carers in planning of care and rehabilitation and promote self-management.
  • Develop enhanced knowledge of community support services.
  • Communicate effectively with the Integrated Community Services team.
  • Provide written reports and verbal feedback on clients’ progress in self-maintenance and leisure.
  • Act as named contact for the client within the department and feedback issues to senior staff.
  • Maintain professional relationships with managers, colleagues, clients and families.
  • Support other Therapy Services departments as needed.
  • Provide advice and teaching to relatives and carers to promote understanding of therapy aims.
  • Undertake holistic therapeutic assessments with diverse conditions increasing fall risk.
  • Hold own caseload and discharge patients; undertake clinical duties as autonomous practitioner.
  • Ensure client records on Systm1 are complete and up to date.

Assistant Practitioner | East London NHS Foundation Trust
  • Works within the multidisciplinary team in a primary care setting including clients’ homes, residential homes and clinic, providing support for a variety of therapeutic and clinical interventions based on the team skill mix.
  • Participate in the assessment, planning, implementation and individualised therapeutic interventions under the guidance of the multidisciplinary team
  • To identify and provide appropriate assistive equipment for clients to facilitate safety in their chosen environment.
  • To work autonomously and undertake lone working in the community To work a variety of shift patterns which include weekends and bank holidays on a rota basis.
  • To travel across the county as service demands require.
  • Responsible for assessing, planning and implementing therapeutic interventions. To assess, demonstrate and provide appropriate, adaptive equipment to meet specific client needs and to facilitate safely in the chosen environment.
  • To form professional relationships with clients, family and carers and communicate condition related information to them in a way that respects their views, autonomy and culture.
  • To involve clients, family and carers in the planning of care/rehabilitation programs and encourage self-management where appropriate. Promoting self-management of health related conditions.
  • To have an enhanced level of knowledge of support services within the community.
  • To communicate effectively with all members of the Integrated Community Services team. To support and promote wider team working.
  • To provide written reports and verbal feedback to the team regarding clients’ performance/progress in areas of self-maintenance, productivity and leisure.
  • To act as named person for the client to contact within the department and to feedback relevant issues to senior staff.
  • Maintain professional, responsible and appropriate relationships with managers, colleagues, clients, their family/friends and other services.
  • To support other departments and localities within the Therapy Services at the request of management as demand requires.

  • To provide advice, teaching and instruction to relatives, carers and other professionals to promote the understanding of the aims of therapy and to ensure a constant approach to patient care.
  • To undertake advanced holistic therapeutic assessments with a diverse range and complexity of physical and psychological conditions which increase the risk of falls.
  • To hold own caseload of clients referred. Taking responsibility for accepting and discharging these patients, undertaking all aspects of clinical duties required within the pathway as an autonomous practitioner.
  • To ensure the client record on Systm1 contains the client’s full journey in a timely manner.
