Job summary

NHS Fife in Kirkcaldy is seeking an assistant practitioner to join Community Rehabilitation Teams. You will work with Occupational Therapy and Physiotherapy staff to deliver therapeutic treatment programmes to service users in their own homes, preventing hospital admission and supporting early discharge from hospital.

Applicants should have an HNC/HND in Health and Social Care or SCQF level 8 and experience as a therapy support worker, especially with older people.

Responsibilities

Job description

An exciting opportunity has arisen for an assistant practitioner to join our Community Rehabilitation Teams in Kirkcaldy.

The Community Rehabilitation Teams provide a service to the adult population within the various localities. This will be delivered to service users within their own home or when deemed essential as an out-patient.

The primary function of this role will be to work with the Occupational Therapy and Physiotherapy staff as part of the Community Rehabilitation Team to deliver therapeutic treatment programmes to service users in their own homes, preventing hospital admission and supporting early discharge from hospital. The post holder will also be expected to hold their own caseload, assess, progress and discharge autonomously within parameters.

You will have an HNC/HND in Health and Social Care or equivalent or SCQF level 8 e.g. Postural Stability Instructor or equivalent. Experience working as a therapy support worker would be essential. It would be essential to have experience working with older people as they are the main group of service users referred.

Working as part of a supportive multidisciplinary team you will be expected to have excellent organisational skills, excellent communication skills and be able to work independently as well as within the team.

There may be occasions where you will be required to work in one of the other community hospital sites.

Public Holiday working is in operation. Our service offers regular supervision, PDP and supports training opportunities. The ability to travel throughout the area is essential as this is a community post.

Interviews will be held face to face.

Bases are to be determined, but will cover throughout Fife and it would be useful to note areas of work preference on the application eg Dunfermline locality/Glenrothes locality.
