Assistant Care Manager Role

Working as an Assistant Care Manager, you will operate in accordance with divisional and departmental policies and procedures to undertake assessments of Adults and Carers, providing practical support and information.

Responsibilities include, but are not limited to:

1. Undertaking assessments of Adults and Carers needs in accordance with the Care Act 2014 and London Borough of Bromley procedures.
2. Preparing individual support plans based on assessments, including identifying unmet needs of Service Users and their Carers.
3. Promoting user and carer choice and independence through Direct Payments/Personal Budgets.
4. Monitoring and reviewing support plans and evaluating the effectiveness of services provided to Adults and Carers.
5. Developing and maintaining knowledge, skills, and expertise in assessment, planning, and review processes.
6. Contributing to service development by providing insights and ideas for future services.

Requirements:

• A relevant social and health care qualification is preferable but not essential.
• Minimum GCSE Grade A-C in English or equivalent.
• Ability to travel independently across various locations within the borough, carrying equipment.
• Excellent communication skills.
• Enhanced DBS clearance, dated within the last 12 months, preferably on the update service. Fast-track options may be available.
