Job summary

St Helens Borough Council in Merseyside invites applications for a part-time Assessment and Review Worker in Contact Cares at Whiston Hospital. The role supports the single point of access for health and social care referrals, with a two-hour response target under Urgent Community Response Criteria.

You'll join a multi-disciplinary team of social care, nursing and therapy staff, providing initial contact, care planning, onward referrals and signposting.

Making A Difference as a part-time Assessment and Review Worker in Contact Cares

An opportunity has arisen for a driven and innovative Assessment and Review Worker on a part-time basis to join Integrated Health and Social Care in the Front Door Service within Contact Cares based at Whiston Hospital, Nightingale House. This large and busy multi-disciplinary team and is the single point of access for health and social care referrals. As the team continues to develop, we are looking to recruit a motivated part-time Assessment and Review Worker to join our team within the service. The team consists of both qualified and unqualified Social Care, Nursing and Therapy staff. We provide a 2-hour response under Urgent Community Response Criteria and offer planned and urgent assessments.

The Contact Cares team also provide initial contact, care planning, onward referrals, and signposting services for a wide range of adult clients across health and social care. With a focus that stretches from prevention and early intervention, to reducing hospital admissions and readmissions the work is varied and interesting.

The team operate from 8.00 am until 10.00 pm 7 days per week to include Bank Holidays. This post will be shift working to a rota of pro-rota, 18.5 hrs per week over 7 days.

Employees can regularly work away from Council workspaces (including home or other locations), alongside required office-based working. The balance between remote and office-based working is determined at service level based on operational need, however, there has been a minimum specification that staff will work at least 2 days from an office-based location.
