We are looking for an enthusiastic and dynamic person to join the Hospital Adult Social Care Team in North Northants Council (NNC). You will be responsible for managing a caseload of individuals; assessing social care needs; developing, co-ordinating and reviewing care support, as well as ensuring that we meet our duty to safeguard vulnerable individuals. You will be committed to working with customers in a person centred way to support them to achieve effective outcomes. You will be supporting people within the community to be able to remain as independent as possible within their own home and reduce the risk of crisis support.
