Job summary

Buckinghamshire Council's Home Independence Service is seeking a Social Care Assistant to support residents in the community, helping them regain independence after illness or disability. You will follow person-centred care plans, deliver practical support, and use technology-enabled care devices where appropriate.

You'll work flexibly in shifts (7am–10pm, every other weekend) and may need a full UK driving licence and access to a vehicle.

Overview

As a Social Care Assistant working within the Home Independence Team, you will be working out in the community to support residents that have been recently discharged from hospital or those who are living with a disability and need help to live independently and establish a routine to help them manage their lives day to day. You will be supporting individuals to regain confidence and reach their goals of living independently again.

We are looking for a dedicated individuals who can provide high quality direct care and support to residents within, focusing on personal wellbeing outcomes and individual wishes, to achieve strength-based support.

Positions available:

We have part time and full time posts available. Please specify your preference within your application form.

Work patterns:

Shift work between 7am to 10pm, 7 days a week. Hours to be agreed with the hiring manager, but will include every other weekend as per rota.

Salary:

£27,939 Assistant Assistant - £29,242 per annum pro rata

About Us

The Home Independence service supports individuals to regain their independence following illness, disability, or recent stay in hospital. We work with clients in their homes, identifying whether they now have difficulties with self-care or performing daily activities. We provide a goal-based assessment for each client and alongside them to support the achievement of their goals, using technology-enabled care devices and occupational therapy aids to enable them to achieve independence.

The team works closely with healthcare professionals, carers, clients and their families to develop suitable packages of support, designed to enhance client independence and meet their needs. We supply advice on specialist equipment options, monitor client progress and make necessary adjustments if required. About The Role

No two days will be the same as you meet a variety of people and support their individual needs. Some of the work you will carry out includes.

• Providing people with information and advice in relation to care and support and assistance in accessing care and support through various means.
• Working as part a CQC Registered service in the community, maintaining a high degree of autonomy and utilising own initiative in delivering compliant care and support independently.
• Ensuring an agreed plan meets individual’s care and support outcomes needs and to empower the individual to access early intervention and preventative services.
• Informing senior colleagues and adhere to safeguarding procedure of any concerns which arise in relation to the safety of vulnerable adults.
• Working in partnership with individuals, carers, service providers, voluntary sector and other professionals.
• Providing personal care for individuals, this may include but is not limited to dressing, toileting, washing and general housekeeping.

About You

Naturally, you will be a great people person, caring and empathetic as well as being able to deliver practical care and support to an agreed plan. You will use your own initiative to problem solve and utilise remote management oversight, where required.

You may need to undertake a specialist role that requires specific training, such as Autism services or undertaking and administering medication. Full training will be provided and there are opportunities to progress within the service.

Other information

Please note, these positions are community based, not day centre based. Therefore a full UK driving licence is essential and having access to a vehicle.

The post holder will be entitled to a car user allowance and a 45p per mile mileage claim allowance (HMRC mileage allowance).
