Supportive Outreach and Engagement Worker (Alcohol Care Teams)
A new opportunity has arisen to join the MFT Alcohol Care Team (ACT), This is an exciting time to join the team, as it expands and develops within the hospitals, and the wider trust.
Supportive outreach and engagement worker position has been created, to support individuals accessing secondary emergency care services, providing a wider, holistic and multi-agency approach to case management.
Facilitating integrated alcohol care between secondary, primary and community services, for acute alcohol frequent hospital attendee’s
This role will be primarily community based, however you may at times be requested to work at one of the 3 Alcohol Care Team locations - Manchester Royal Infirmary, Wythenshawe Hospital or North Manchester General Hospital.
The focus of the role would be working with high intensity users of the trust, who present to the Emergency Departments or are admitted in relation to alcohol misuse. Ensuring effective implementation of care pathways and being the interface between community substance misuse services and the Alcohol Care Teams. Providing the extension of care beyond discharge, to support individuals who may be resistant to services, hard to engage, or treatment naïve at point of discharge. The post holder will attempt to re-engage individuals who have dropped out of treatment or detached from services
