A new opportunity has arisen to join the MFT Alcohol Care Team (ACT). This is an exciting time to join the team as it expands within the hospitals and the wider trust.
The Supportive Outreach and Engagement Worker role has been created to support individuals accessing secondary emergency care services, providing a holistic and multi-agency approach to case management.
This role facilitates integrated alcohol care between secondary, primary, and community services for acute alcohol hospital attendees.
The position is primarily community-based, but you may be required to work at one of the three Alcohol Care Team locations: Manchester Royal Infirmary, Wythenshawe Hospital, or North Manchester General Hospital.
The focus is working with high-intensity users of the trust who present to Emergency Departments or are admitted due to alcohol misuse. Responsibilities include implementing care pathways and acting as an interface between community substance misuse services and the Alcohol Care Teams. The role extends care beyond discharge to support individuals who may be resistant or difficult to engage, including those who are treatment-naive at discharge. You will also attempt to re-engage individuals who have dropped out of treatment or detached from services.
