Join to apply for the Advanced Social Work Practitioner role at London Borough of Hillingdon

About the role

We are seeking an enthusiastic individual to join our hospital team in managing safe and efficient hospital discharges. This role operates in a fast-paced environment where delivering results and meeting performance targets are crucial.

Key Responsibilities:
  1. Multi-Disciplinary Assessments: Conduct assessments with health professionals, including DSTs.
  2. Duty Management: Oversee daily discharges via the D2A pathway and Pathway 3 placements, triage cases with the Hillingdon HUB, and manage out-of-borough hospital referrals. Participate in conference calls with health professionals to facilitate decision-making.
  3. Staff Supervision: Allocate cases, manage stepdown beds, and supervise staff to ensure smooth team operations.
  4. Safeguarding Management: Attend weekly meetings with the safeguarding lead to monitor investigations.
  5. Case Management: Handle a small caseload of complex cases, providing expertise and oversight as needed.
Requirements:

The candidate must have completed a Practice Educator Training Qualification.
