We are enhancing our Urgent Community Response Team and have opportunities within the team for a senior clinician. The post holder will be a senior member of the team. The post holder will work autonomously and in collaboration with Acute Trusts, the Adult Community Nursing Team, other healthcare professionals, and statutory and voluntary agencies to deliver excellent standards of patient care.

The successful candidate will be an experienced practitioner with sound clinical knowledge and proven experience in acute and community clinical environments. You will need to be adaptable to operate in a fast-moving environment, providing unscheduled holistic clinical care to patients presenting with acute undifferentiated and undiagnosed conditions, complex health needs, and/or long-term conditions.

Main duties of the job

As a senior practitioner, you will act as a role model, supporting the Urgent Community Response team to achieve and maintain high standards of clinical care based on evidence-based practice.

You will support the clinical lead in responding to clinical incidents, accidents, and complaints in accordance with CLCH policy.

You will work in collaboration with and maintain good working relationships within the other Community Adult Health Care Teams within Harrow and adjoining boroughs, as well as with the District Nursing team, Out of Hours Services, A&E Departments, London Ambulance Service, Acute Trust, Community Matrons, General Practitioners, Practice staff, Nursing Homes, Social Services, and Voluntary Sector to provide comprehensive patient care.

You will undertake assessment and appraisal of designated team members.

Person Specification
Education/Qualification
  • Current registration as a healthcare professional (Nurse or similar)
  • BSc / Pg Dip Nurse Practitioner in Primary Health Care, Minor Injuries, or Minor Illnesses
  • Experience in Community and/or Accident and Emergency
  • Evidence of ongoing professional education at master's level
  • Desirable qualifications: Primary Care, Accident & Emergency, or Advanced Clinical Practitioner
  • Qualification in teaching or practice-based learning
  • Independent prescriber or willingness to undertake
Experience
  • Extensive experience in Community, Primary Care, and/or A&E
  • Proven experience utilizing diagnostic and clinical skills for autonomous unscheduled care
  • Experience caring for people with Long Term Conditions
