Job summary

Kernow Health CIC is a Community Interest Company delivering NHS urgent care services across Cornwall, with the 111 Integrated Urgent Care Service and other programmes. The primary base is Cudmore House, with clinical work across treatment centres or home visits.

A mix of day, evening and night shifts supports a 24/7 operation. Advanced practitioners must attend multiple clinical sites in line with need, and staff benefit from NHS Pension and a multidisciplinary, forward-thinking team.

Qualifications

• Autonomous Clinical Practice: Deliver high-quality urgent care assessment and management for patients across the 111 Clinical Assessment Service, face-to-face treatment centres and home visiting settings.
• Clinical Decision-Making: Undertake comprehensive history taking, examination and clinical assessment; formulate management plans and ensure safe follow-up.
• Integrated Working: Work with GPs, Clinical Shift Supervisors, Care Navigators and the multidisciplinary team to maintain patient flow.
• Clinical Governance & Quality: Contribute to governance, audits, incident reporting and service improvement.
• Escalation & Risk Management: Recognise deteriorating patients and elevate complex cases with sound judgement.
• Safeguarding & IPC: Maintain safeguarding vigilance and IPC standards across environments.
• Digital & Documentation Standards: Ensure accurate documentation across multiple clinical systems.

Responsibilities

• Digital & Documentation Standards ensures accurate, contemporaneous documentation across systems.

Shift Patterns Available

Option 1: 40 hours per month available as part of a fixed or flexible four week rolling rota working weekends.

Option 2: 15-37.5 hours per week working within a relief role with contracted hours and covering service gaps during evenings and nights. (A minimum of four weeks’ notice of shifts is provided with relief working in this role)

Salary

£40.42 / £42.40 / £44.38 per hour base rate (Level 1 / 2 / 3 dependent on experience and qualifications) plus unsocial enhancements in line with service requirements.

Permanent

The primary base is Cudmore House, with clinical work allocated across treatment centres or home visiting cars throughout Cornwall. Advanced Practitioners must be able to attend Cudmore House and other clinical sites in line with operational need.

There are a mixture of daytime, evening and overnight shifts available however as an out-of-hours predominant service the vast majority of the rota occurs between 1830 0800 Monday Friday and 24/7 across weekends.

Main duties of the job

Autonomous Clinical Practice

Deliver high-quality urgent care assessment and management for patients across the 111 Clinical Assessment Service, face-to-face treatment centres and home visiting settings. Practise safely and independently within your scope of competence.

Clinical Decision-Making

Undertake comprehensive history taking, examination and clinical assessment. Formulate management plans, prescribe where appropriate, and ensure safe follow-up or onward referral.

Integrated Working

Work collaboratively with GPs, Clinical Shift Supervisors, Care Navigators and the wider multidisciplinary team to ensure safe and efficient patient flow across the IUCS pathway.

Clinical Governance & Quality

Adhere to governance processes including incident reporting, audit participation and reflective practice. Contribute to maintaining high clinical standards and service improvement initiatives.

Escalation & Risk Management

Recognise and appropriately elevate deteriorating patients or complex cases. Support system flow through sound clinical judgement and safe risk assessment.

Safeguarding & IPC

Maintain safeguarding vigilance for adults and children and adhere to Infection Prevention and Control standards across all clinical environments.

Digital & Documentation Standards

Demonstrate strong digital literacy, ensuring accurate, contemporaneous and high-quality documentation across multiple clinical systems.
