Job summary

Taurus Healthcare is seeking an Advanced Practitioner (ACP/ANP/Specialist Paramedic) to join our expanding Out of Hours service in Herefordshire. You will deliver urgent primary care with autonomy, including telephone triage, face-to-face assessments, home visits and prescribing under GP support.

We welcome experienced ACPs, ANPs and Specialist Paramedics who can work independently within a multidisciplinary team and contribute to service development, safety and quality improvement.

Qualifications

  • Postgraduate Level 7 clinical qualification or working towards it.
  • Registered Nurse (NMC) or Paramedic (HCPC).

  • Provide advanced clinical assessment, diagnosis and treatment for patients of all ages.
  • Manage undifferentiated and complex presentations in an urgent care setting.

Advanced Practitioner (ACP/ANP/Specialist Paramedic)

We are looking for full and part time experienced Advanced Practitioners to join our expanding Out of Hours service at Taurus Healthcare.

This is an exciting opportunity to become part of a supportive multidisciplinary team delivering urgent primary care across Herefordshire. We welcome applications from

  • Experienced Advanced Clinical Practitioners
  • Advanced Nurse Practitioners
  • Specialist/Advanced Paramedics

who are confident working autonomously with patients presenting with undifferentiated urgent care needs.

Working across our Out of Hours service in Hereford, you will provide telephone triage, face-to-face consultations within our treatment centres and home visiting, supported by experienced GPs and a multidisciplinary clinical team. You will independently assess, diagnose, treat and prescribe for patients of all ages, helping deliver safe, high-quality urgent primary care when patients need it most.

Due to current licence limitations, visa sponsorship is not available for this position. Applicants must already hold the right to work in the UK.

Job responsibilities

JOB PURPOSE:

The Advanced Clinical Practitioner (ACP) is a senior autonomous clinician who delivers high quality, evidence based care across Taurus Healthcare's integrated primary care and urgent care services, supporting the organisations vision of accessible, 24/7 general practice.

The post holder will assess, diagnose, treat and manage patients with undifferentiated, acute and complex conditions, while contributing to service development, clinical leadership, education and quality improvement across the system.

Main Responsibilities

1. Clinical Practice

Practice as an independent and autonomous clinician, managing patients with undifferentiated, acute and chronic conditions across face to face, home visiting, remote and digital consultation models

Undertake comprehensive clinical assessment, diagnosis and management, demonstrating advanced clinical reasoning, risk assessment and safety netting

Initiate, interpret and act on appropriate investigations and referrals in line with local and national pathways

Prescribe and manage medications safely and effectively in accordance with national guidance, local formularies and EMIS decision support tools

Contribute to flow, continuity and demand management across integrated services, including prevention of unnecessary hospital admissions

Respond confidently to complex, urgent and emergency presentations, initiating immediate care as required

Safeguard children, young people and adults, recognising risk, acting within statutory frameworks and escalating concerns appropriately

2. Integrated & Multidisciplinary Working

  • Work collaboratively with GPs, nurses, paramedics, pharmacists, operational teams and external partners to deliver seamless, patient-centred care

Support system wide working by ensuring patients receive the right care, in the right place, at the right time

Contribute to the development of new models of care that enhance access, quality and patient experience

Act as a senior clinical decision maker within the multidisciplinary team

Provide visible clinical leadership, supporting safe practice, innovation and continuous improvement

Contribute to service redesign, pathway development and quality assurance activity

Support the investigation and learning from clinical incidents, audits and significant events

Promote a positive learning culture, role modelling professionalism and reflective practice

Personal Qualities or attributes
  • Commitment to integrated, patient-centred care and service innovation
Qualifications
  • Relevant postgraduate clinical qualification at Level 7 (or working towards) qualification or equivalent
  • Registered Nurse (NMC) or Paramedic (HCPC)
Skills
  • Proven ability to work autonomously while recognising professional limits
  • Excellent communication, clinical reasoning and decision-making skills
  • Driver with clean licence
Experience
  • Minimum 5 years post-registration experience, including significant autonomous practice.
  • Demonstrable experience managing undifferentiated and complex clinical presentations.
  • Experience working across primary care and urgent care settings.
  • Experience of EMIS and remote consultation platforms.
  • Experience of supporting education, supervision or mentoring
Skills
  • Proven ability to work autonomously while recognising professional limits
