We are looking for an experienced and motivated Advanced/ Senior Practitioner to join our 18 –25 Transition Team. You will hold a manageable caseload alongside providing professional guidance and supervision to social workers and other team members. You will support the Team Manager with the day-to-day running of the service and provide management cover when required. You will bring strong knowledge of the Care Act 2014, safeguarding adults, the Mental Capacity Act, Court of Protection (CoP) and Deprivation of Liberty Safeguards (DoLS), supporting high-quality, legally compliant practice. This is a part time role so please note the salary will be pro rata. The role involves working two days a week. Monday is a mandatory working day in person at Sand Martin House. Interview will take place on Monday 23rd October.
Our Transition Team supports young people as they prepare for adulthood, working closely with families, carers, education, health and other professionals. We are a supportive and collaborative team committed to achieving positive outcomes for young people while promoting independence, choice and control. As a Senior Practitioner, you will play an important role in supporting consistent practice, developing staff and contributing to continuous service improvement.
