Job summary

Promni Health in England is seeking an Advanced Practice Physiotherapist (APP) to join our NHS-aligned GP service. You will assess, diagnose, triage and manage complex MSK cases within primary care, and you may use independent prescribing where appropriate.

This role collaborates with PCNs, supports service development, and contributes to training, audits and research to continuously improve patient pathways.

Qualifications

• Fully HCPC and CSP registered practicing Physiotherapist (essential).
• MSc in Advanced Physiotherapy Practice (essential).
• Post-graduate Non-Medical Independent Prescribing (essential).
• Post-graduate Soft Tissue & Joint Injection Therapy (desirable).

Role Profile

Advanced Practice Physiotherapists (APP) are qualified autonomous clinical practitioners working in GP practices who can assess, diagnose, treat, and manage musculoskeletal (MSK) problems and undifferentiated conditions, and where appropriate, to discharge a person without a medical referral. APPs working in this role can be accessed directly by self-referral or staff in GP practices can direct patients to them to establish a rapid and accurate diagnosis and management plan to streamline pathways of care.

Key Responsibilities

Clinical Practice

• Assess, diagnose, triage and manage patients taking responsibility for the management of a complex caseload. You will progress and request investigations (such as x-rays, and blood tests) and referrals to facilitate diagnosis and choice of treatment.
• Manage Musculoskeletal patients by making full use of independent prescribing skills.
• Work in partnership with the Primary Care Network and the practices to develop and implement specialist musculoskeletal services to triage patients before referring for secondary care opinion.
• Develop and make use of your full scope of clinical practice. Develop and conduct a comprehensive assessment of patients with chronic pain, complex presentations / multiple pathologies (for example, persistent spinal pain, neuropathic pain, fibromyalgia, complex regional pain syndrome), using highly advanced assessment skills including medication history, prescribing, de‑prescribing methods, injection therapy and investigation.
• Utilising highly effective clinical reasoning and manual assessment skills to provide an accurate diagnosis and formulate a specialised programme of care for patients presenting with complex musculoskeletal conditions.
• To develop collaborative working pathways with an interdisciplinary approach with the community and secondary orthopaedic, pain management teams.
• Manage complex interactions, including working with patients with psychosocial and mental health needs, referring to social prescribing when appropriate.
• Deliver programmes of supported patient self-management, in ways that facilitate behavioural change, optimise individuals’ physical activity, mobility, fulfilment of personal goals and independence, and that minimise the need for pharmacological interventions.
• Provide care which is tailored to the individual needs. This would include, appraising the impact pf individuals’ clinical status on their general health, well-being, employment status (including in relation to function, physical activity, mobility, and independence).
• Support patients to set their own goals and be confident in their approach to self management.
• Communicate effectively and appropriately, with patients and carers, complex sensitive information regarding diagnosis, pathology, prognosis and treatment choices supporting personalised care.

Evidence, Research, and Development

• Collect, analyse and present data on patient case-mix, produce information on patient’s needing specialist care, metrics of interventions such as auditing Injection therapy , use of prescribing, conversion of secondary care referrals.
• Auditing on complex drug management including prescribing, de‑prescribing in chronic widespread musculoskeletal pains coupled with mental health and other co‑morbidities.
• Critical analysis of data to draw meaningful recommendations to the Primary Care Network to inform best practice, influence good clinical practice in primary care.
• To support the development and implementation of a training strategy for area of responsibility in collaboration with team leads/clinical academics, and clinical specialists internally and outside the Organisations.
• Participate in ongoing service improvement, evaluation and audit of interventions, outcomes as appropriate to context and management
• Develop relationships and a collaborative working approach across the PCN supporting the integration of pathways in primary care.
• Work as part of an MDT in a patient facing role, using your expert knowledge of MSK issues, to create stronger links for wider MSK services through clinical leadership, teaching and evaluation skills.
• Support research and audit programmes to evaluate and improve the effectiveness of our primary care MSK service.
• Streamline the pathway of care by providing a responsive service so that patients receive timely access to care.
• Develop Standard Operating Procedures (SOPs) appropriate for onward referral pathways.

Leadership & Line Management

• Contribute to the teaching and training of other healthcare staff re: pain management, musculoskeletal assessment to including other physiotherapists, doctors, nurses and other health care team members in Primary Care Networks.

Facilitating learning

• Provide feedback and support to other PCN clinical team members on appropriate referral criteria.
• Critically reflect on own practice, and continue to develop your skills.
• Support FCP colleagues in line with Health Education England’s ‘First Contact Practitioners and Advanced Practitioners in Primary Care: (Musculoskeletal). A Roadmap to Practice’.
• Be a point of contact for your FCP colleagues to answer clinical queries as and when they present.

Experience, Knowledge And Qualifications

• Fully HCPC and CSP registered practicing Physiotherapist (essential)
• An up-to-date CPD portfolio demonstrating that you have knowledge of the musculoskeletal. core capabilities framework for first point of contact practitioners and the four pillars of advances practice, in addition to identifying your developmental needs against these (essential)
• Extensive clinical experience of delivering musculoskeletal assessment and treatment which should be at a Band 8a + level (essential)
• Completion of an appropriate level 7 MSc in Advanced Physiotherapy Practice (essential)
• Completion of M-level post-graduate Non-Medical Independent Prescribing course (essential)
• Completion of M-level post-graduate Soft Tissue and Joint Injection Therapy course (desirable)
• Previous experience of using delivering FCP services in primary care (desirable).
