Job summary

Lister House Surgery seeks a first contact physiotherapist practitioner to join its forward-thinking multidisciplinary team. The post holder will manage a complex MSK caseload, work independently, and have opportunity to become an advanced practitioner with a focus on special interests.

You will receive clinical supervision from Clinical Directors and support from the PCCO PCN, with opportunities to lead on identified services and projects, while collaborating across GP practices and community

Job Summary

An exciting opportunity has arisen for a first contact physiotherapist practitioner to join our forward-thinking, multidisciplinary team and support PCCO Primary Care Network. We have a well-developed MSK team, comprised of three GPs with a specialist interest in MSK and three senior physiotherapists. The post holder will manage a complex caseload of patients, working independently to assess, diagnose and manage patient's musculoskeletal problems. This physiotherapist will be given the opportunity to become an advanced practitioner using their own special interests, a clinical case load and being hands on in group work. At PCCO PCN, you will be given the agency to work as an independent professional, working with the PCN management team to analyse both qualitative and quantitative data to identify areas of need within our population and formulate plans and projects to create support pathways for these patients. You will also receive referrals from other PCN and practice members to provide 1-2-1 support to patients who require an MSK assessment. You will have access to clinical supervision from one of our Clinical Directors and support from the rest of our PCN and practice teams. You will work closely with GPs, community services, the city MSK service, EMAS, Social Care, Practice staff and wider MDTs.

Main duties of the job

The post holder will be given the autonomy to specialise in specific health conditions in line with population health analysis and work with the wider MDT for conditions such as chronic pain. If you have a specialism, such as chronic pain, chronic back pain or in tackling health inequalities, this will be considered as an advantage, and we would love to hear about what you believe you could personally bring to our PCN.

Job Description/Responsibilities

• Assess, diagnose, plan and implement specialised therapy programmes using advanced level analytical and judgement skills and expert knowledge.
• Order further investigations and interpret the results of diagnostic tests e.g., MRI, X-Ray, nerve conduction or blood tests.
• Identify patients' capacity to consent to treatment and apply the consent protocol.
• Provide highly specialised, evidence-based advice regarding the care and treatment of patients.
• Work as an autonomous practitioner, providing a highly specialist level of physiotherapy care, making onward referrals to other professionals.
• Offer specialist advice to assist in the implementation of service improvements.
• Be flexible to the needs of service delivery.
• Contribute to the clinical education of the multi-disciplinary team within the Partnership.
• Be responsible for personal clinical record keeping in accordance with the CSP guidelines and standards for documentation and legal requirements.
• Review & improve the quality of the service through regular appraisal of research articles & journals and implementation of evidence-based practice.
• Develop evidence-based patient information leaflets.
• Implement clinical change regarding policies & practice, in response to national & local clinical guidelines and emerging evidence.
• Communicate complex information with empathy & consideration to patients and carers. This may relate to patients’ diagnosis, prognosis, treatment plans and expected goals, therefore could be highly sensitive or distressing and associated with high degrees of emotion or conflict.
• Use effective communication skills to gain co-operation with treatment where there may be significant barriers to understanding or ability to co-operate with the treatment.
• Complete comprehensive, accurate and legally compliant clinical records or reports in accordance with professional standards.
• Work effectively with others to clearly demonstrate Lister House values, direction and policies.
• Discuss, highlight and work with the team to create opportunities to improve patient care.
• Manage and lead on the delivery of specifically identified services or projects as agreed with the practice management team.

• Act as an expert knowledge resource for colleagues including GPs, nurses and other AHPs.

• Participate in research and development activities, linking with regional and national initiatives and disseminate, audit & research findings at all levels e.g., local, regional and national where appropriate.
• The post-holder must always comply with the Practice’s Health and Safety policies, report incidents using the Incident Reporting System.
• The post-holder must undertake statutory and mandatory training in line with the Organisation’s requirements annually.

Qualifications

Essential

• Registered with Health Care Professions Council (HCPC)
• BSc (Hons) Physio or equivalent
• Able to demonstrate in practice portfolio experience in core physiotherapy and MSK physiotherapy
• Member of the Chartered Society of Physiotherapy (CSP)

Desirable

• May hold or be working towards a prescribing qualification
• Basic level management/leadership training
• May hold or be working towards an injection qualification

Experience

Essential

• Demonstrates understanding of, and conforms to, relevant standards of practice
• Minimum of 5 years clinical experience.
• Solid base of specialised experience in relevant areas.
• Understanding of the implications of national priorities for the team and/or service
• Understanding of the process for effective resource utilisation
