Qualifications

• Excellent communication skills, both written and verbal.
• Strong IT skills including common software and email tools.
• Clear, polite telephone manner and professional presentation.
• Ability to record accurate clinical notes and manage documentation.
• Good interpersonal and teamwork abilities; autonomous when required.
• Ability to follow clinical policy and procedures with attention to detail.
• Experience with primary care IT systems and electronic records.

Responsibilities

• Work as an autonomous practitioner within the GP team.
• Deliver health promotion and chronic disease management.
• Review and manage patients' medicines and medication reviews.
• Support safeguarding and liaise with external services as needed.
• Undertake home visits for housebound patients when required.
• Maintain accurate records and contribute to audits.
