This is an exciting opportunity for an experienced Advanced Nurse Practitioner with a special interest in frailty/older peoples health to develop their clinical practice in a leadership role.

You will work directly with a small team including a community Paramedic and Care Co-ordinators to support care home residents and frail older people in their own homes. You will ensure PCN and member Practices meet their requirements to provide enhanced health in care homes, and work with care homes to develop and promote good practice to maximize residents' health and quality of life.
You need to be registered as a Nurse with the Nursing and Midwifery Council and hold an appropriate Advanced Practice qualification (post-grad diploma, degree, or Masters if qualified after 2020). You should also hold or be working towards a prescribing qualification.

The purpose of this role is to support PCN staff and member Practices in delivering high-quality care to frail elderly people and residents in nursing and care homes. As an Advanced Practitioner, your job plan will include research, education, leadership roles, and direct patient care.
You will be working with five GP Practices, each offering high-quality patient care, as part of a wider PCN team including Pharmacists, Physios, and Social Prescribers.
We offer a competitive salary: £48,526 - £54,619 (qualified prescriber, dependent on experience). Candidates working towards but not yet completed their Prescribing Qualification would start on £43,806 - £47,672.
Working Hours: Up to full-time 37.5 hours (minimum 22.5 hours).
Pattern: Flexible within Network Standard Hours (Monday to Friday, 8:00-20:00, occasional Saturday work).
