We are looking for an experienced ANP to work with us for a minimum of 30 hours a week.
JobSummary:
We are seeking a highly skilled and motivatedAdvanced Nurse Practitioner (ANP) to join our team as the Lead Nurse. Thesuccessful candidate will be responsible for providing high-quality care,including the management of minor ailments, chronic diseases, prescribingmedications, womens health and leading the nursing team to ensure the deliveryof exceptional patient care.
We are a strong multi-disciplinary team comprising of 13 GPs, 1 ANP, 2 clinical pharmacists, pharmacy technician, mental health practitioners, practice nurses, HCAs and a full management and administrative support team. We are recruiting due to our existing ANP moving on to new adventures. We work from new premises, fully completed in July 2024, with a beautiful staff room.
The team has the advantage of working closely with East & Central Brighton PCN Community Interest Company, providing additional support to our patients.
We offer a Bike to Work scheme, free bus transport within the first 4 weeks of taking up the post, weekly clinical meetings, regular social events, Balint groups and plenty of time for reflective practice. We promote a healthy working environment and support the wellbeing of each other.
We support first year medical students, FY2 doctors and will very shortly become a full training practice.
ClinicalPractice
Conductautonomous comprehensive assessments, diagnoses, and treatment plans forpatients with minor ailments.
Manageacute and chronic conditions.
Requestand interpret diagnostic tests.
Prescribemedications in accordance with legal and professional guidelines.
Provideexpert clinical advice and support to patients, families, and carers.
Collaboratewith GPs and other healthcare professionals to ensure integrated patientcare.
Providehealth promotion and disease prevention advice
Beknowledgeable in the area of women's health
Implement and evaluate individual specialised treatment plans for chronic disease patients.
Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects on the patients' health
Leadership& Management
Lead and manage the nursing team, providing guidance, support, andmentorship.
Work with the management team to ensure all targets are met wheneverpossible.
Oversee the development and implementation of nursing policies andprocedures.
Ensure compliance with clinical governance and quality standards.
Participate in recruitment, training, and performance management ofnursing staff.
Participate in clinical audits when necessary
Delegate clinical responsibilities appropriately, ensuring safe practiceand the task is within the scope of practice of the individual
Health Improvement
Management of health screening programmes and ensures other teammembers are aware of health priorities and screening objectives and programmes.
Innovation in practice to meet ongoing demands on the practicesdelivery of care.
Improve health outcomes for patients by introducing newdevelopments, and can demonstrate them as reports and presentations whereappropriate.
Supports practitioners in reporting the practice population topromote health improvement and disease prevention.
Makes referrals to appropriate agencies (statutory and voluntary)where necessary
Education& Development
Promote continuous professional development and education within thenursing team.
Lead and participate in clinical audits, research, and quality improvementinitiatives.
Provide training and education to patients and staff on health promotionand disease prevention.
Maintain clinical competence through reflective practice and appraisal.
