Job summary

The Village Surgeries is seeking a motivated Advanced Nurse Practitioner to join our two-site practice at Croston and Eccleston. You will work autonomously, diagnosing, managing, prescribing and discharging patients across all ages, including children, via face-to-face or telephone consultations.

Qualifications

• Current NMC registration is required.
• Advanced Clinical Practice/ANP qualification or equivalent.
• Significant post-registration clinical experience is required.
• Experience with chronic disease management is required.

Responsibilities

• Assess, diagnose and manage patients presenting with undifferentiated and acute conditions.
• Undertake clinical examinations and request and interpret appropriate investigations.
• Develop and implement appropriate treatment and management plans.
• Prescribe safely within your scope of practice and prescribing qualification.
• Provide same-day and urgent care where appropriate.
• Recognise and appropriately manage patients who are acutely unwell, escalating to a GP or emergency services when required.
• Provide appropriate referrals and safety-netting advice.
• Maintain accurate and timely clinical records.
• Work collaboratively with GPs and the wider multidisciplinary team.
• Contribute to clinical governance, audit, quality improvement and practice development.
• Participate in clinical meetings, supervision and continuing professional development.
• Complete home visits.
• Demonstrable experience of autonomous clinical assessment, diagnosis and treatment.
• Excellent communication and clinical decision‑making skills.
• Ability to recognise limitations of competence and seek appropriate advice.
• Experience working in General Practice.
• Experience of autonomous clinical assessment and decision‑making.
• Experience managing acute and long‑term conditions.
• Knowledge of EMIS.
• Experience leading service improvement or audits.

Main duties of the job

The successful Advanced Nurse Practitioner will work as an autonomous practitioner, diagnosing, managing, prescribing, and discharging patients with illness or injury across all age groups including young children. This may be by face‑to‑face consultation, or telephone.
